BMP Trainer Travel Form – Air

Please fill out the following information to assist us in coordinating your travel arrangements for the Burn Management Program (BMP).

Travel guidelines:

• Travel from Perth return only.
• The Fiona Wood Foundation will book the cheapest fare/carrier, considering the BMP program schedule.
• Depending on the location, BMP includes flights to and from the location within the same day (i.e., no accommodation is provided unless pre-approved for extended visits).
• Taxi vouchers – if required – limit two per person, plus 2 for the team of four from the airport to the BMP venue and return.
